This program hosted by South-East Ottawa Community Health Centre offers free, one-on-one support to help people of all ages improve their health, prevent illness and live well.The program offers:Health educationChronic disease risk screening (cancer and diabetes)Personalized health coachingHelp connecting to primary care and community servicesFollow-up and ongoing supportThis program is open to anyone living in Ottawa who is interested in improving their health. People can join and leave the program at any time. Program services are offered in English, French, and Arabic.
