An outreach team from Centre 507 will be at Main (1st floor) every Monday from 10:30 AM to 3:00 PM.Harm Reduction Support Provide harm reduction information, education, and supplies, including naloxone kits, safer substance use resources, and referrals to addiction treatment and other social services. Support individuals in accessing services that help improve their health, safety, and well-being.Referrals and Client Support Connect individuals with shelters, housing programs, addiction treatment, mental health services, and other community resources. Work closely with local health and social service organizations to help clients access the care and supports they need, including booking appointments on their behalf when appropriate. Follow up with individuals to encourage ongoing engagement with services and support their progress toward personal goals.Community Outreach and Collaboration Build positive relationships with local businesses, residents, and community partners. Provide education on harm reduction practices and promote strategies that support community safety and well-being. Collaborate with stakeholders to address community concerns and strengthen connections to local resources.***Deux membres de l’Équipe d’intervention du Centre 507 seront présent la succursale Centrale au 1er étage les lundis de 10h30 15h.Réduction des méfaits Fournir de l’information sur la réduction des méfaits, des fournitures, comme des trousses de naloxone, des ressources pour une consommation sécuritaire et un aiguillage vers des services de traitement des dépendances et d’autres services sociaux. Aider les clientes et clients obtenir des services qui contribueront améliorer leur santé, leur sécurité et leur bien-être.Aiguillage et soutien Mettre les clientes et clients en contact avec des refuges, des programmes de logement, des services de traitement des dépendances, des services de santé mentale et d'autres ressources communautaires. Travailler étroitement avec les organismes locaux de santé et de services sociaux pour aider la clientèle obtenir les soins et le soutien nécessaires, y compris prendre des rendez-vous pour les personnes qui en ont besoin. Faire des suivis pour encourager les clientes et clients poursuivre avec les services offerts et les aider atteindre leurs objectifs personnels.Rayonnement et collaboration communautaires Établir des relations positives avec les commerces locaux, la population et les partenaires communautaires. Fournir de l’information sur les pratiques de réduction des méfaits et promouvoir des stratégies de sécurité et de bien-être communautaires. Collaborer avec les parties prenantes pour répondre aux préoccupations de la population et pour renforcer les liens avec les ressources locales.
